Refer a Child to Attain SLB
Pediatricians, therapists, school staff and family members can refer a child to Attain SLB in about 2 minutes. We serve children ages 1 to 18 with in-home ABA therapy across Colorado and at our clinic in Fort Collins.
Who can refer
Pediatricians and other doctors, therapists, school staff, and family members. If you know a child ages 1 to 18 in Colorado who could benefit from ABA therapy, please let the family know and send us their contact details.
What happens next
No waitlist. We work with children ages 1 to 18, and there’s no commitment required.

Pediatricians and other doctors, therapists, school staff and family members can all use this form. Please make sure the family knows you are making the referral.
We work with children ages 1 to 18. We provide in-home ABA therapy across Colorado, plus ABA therapy at our clinic at 1224 E Elizabeth St in Fort Collins.
We accept Aetna, UMR, Health First Colorado (Medicaid), Cigna, BCBS and Optum. We don’t accept Tricare. If you’re not sure about the family’s plan, choose “Not sure” and we’ll check their benefits with them.
No, please don’t. We only need contact details to reach the family. We’ll talk through everything else with the family directly. There’s no waitlist and no commitment required.
